We are always glad to meet new patients! For your first visit, please arrive at least 15 minutes prior to your appointment so we can ensure that we have acquired the necessary information. You will need to bring your insurance card and another form of identification. Please do not wear make-up so we may provide a complete examination.

Additionally, we now have an electronic medical record system which will allow you to create a secure online account with our office. Creating an online account will allow you to send us messages, request refills on prescriptions, obtain patient education materials, and view and pay your account statements. First, please call our office and schedule an appointment. We will send you an email which will allow you to set up a username and password for our patient portal. If you should need to cancel your appointment, we request that you call 48 hours in advance so that we may offer that time to another patient.
We accept most major insurance plans, including Medicare and Tricare. We do not accept Medicaid. If you are unsure if we take your insurance, please contact your insurance company to verify that we are an in-network provider. If your insurance requires a referral to see a specialist, it is your responsibility to see that the referral is sent to us by your primary care provider before your appointment.
Methods of Payment:
If you plan to pay your co-pay or visit fee in cash, we appreciate it if you would bring exact change. We also accept personal checks as well as Visa and MasterCard.
